  <p>Information: Hershel J. Maddox, 63, of 2218 Fairfax Av., died yesterday morning in Camden-Clark Memorial Hospital following an extended illness.  Born in Sandyville, Jackson County, he was a son of Mrs. Bertha Smith Maddox of Sandyville and the late John Maddox.  He had been an employee of the FMC Corp. for the last 41 years and was a member of the Lockhart Methodist Church.  Survivors in addition to his mother include three half-brothers, Harry and Holly Maddox, both of Sandyville and George Maddox of Barlow, O.; five half-sisters, Mrs. Vesta Pickens of Sandyville, Mrs. Pearl Wilson of Parkersburg, Mrs. Mae McCauley and Mrs. Marie Treadway both of Marietta and Mrs. Martha Casperson of Whipple, O.; and several nieces and nephews.  Funeral services will be held at 11 a.m. Monday at the Franklin and Son Memorial Chapel, 2711 Camden Av., with the Rev. H. E. Bailey officiating.  Burial will be in IOOF Cemetery. </p>
