  <p>Information: Mrs. Mary Agnes Kolanko Kwasnik, 68, of 401 Crooks Ave., Ravenswood, died Sunday in Jackson General Hospital following a short illness.  Born in Pittsburgh, Pa., a daughter of the late Mr. and Mrs. Stanley Kolanko, she was a member of Blessed Sacrament Catholic Church in Ravenswood. She was a former resident of Weirton.  Survivors include her husband, John; one daughter, Jean Dysarovich of Arlington Heights, Ill.; one son, John, Jr. of Jackson, N. J.; two sisters, Helen Blank of Weirton and Laura Schmidt of Rilato, Calif.; one brother, John Kolanko of Pittsburgh; two half-sisters, Minnie and Irene; two half-brothers, Harry and Edward; and one granddaughter.  Funeral Mass will be celebrated at 10 a.m. Thursday at Blessed Sacrament Church with Father Joseph Mascioli, celebrant.  Burial will be in Ravenswood Cemetery.  Christian wake services will be recited at 8 p.m. today, (Wednesday) at Chambers Funeral Home, where friends may call.</p>
